Part D Potential Exclusion Warning Report

CMS is posting file layouts for the Part D Potential Exclusion Warning Report and the Part D Exclusion from Reconciliation Report to the CSSC website at Part D Potential Exclusion Warning Report.  An explanation of the reports can be found in the January 3, 2014 Health Plan Management System memorandum titled, “Reconciliation PDE Exclusion Process”.  Questions about the file layouts and the guidance should be sent to CMS at PDEJan2011@cms.hhs.gov.
